Strengthening the Trenton Health Team to Become a Medicaid Accountable Care Organization

The Trenton Health Team (THT) is a community health improvement collaborative serving the six zip codes of Trenton, New Jersey. The collaborative is a partnership between St. Francis Medical Center, Capital Health, Inc., Henry J. Austin Federally Qualified Health Center, and the Department of Health of the City of Trenton.

This project supports several key activities that have enhanced the THT’s ability to become a successful Medicaid Accountable Care Organization (ACO), including expanding care coordination for high users of avoidable hospital services, implementing a Health Information Exchange, and strengthening THT’s organizational infrastructure. On June 30, 2015, THT, along with its partner organizations, was one of three community healthcare coalitions certified by the State of New Jersey as a Medicaid ACO.
