Our Goal: To reduce health disparities and improve the health and well-being of residents of the greater Freehold, NJ area.

Project Description: This grant will provide continued support to The Freehold Neighborhood Connections to Health Coalition (NCHC), a collaborative effort of community organizations aimed at improving the health of the underserved in New Jersey’s Freehold Borough. The grant funds will be used to strengthen the organization, undertake strategic planning, and support ongoing efforts to reduce the incidence of elevated lead levels in the blood of the community’s children and protect them from further exposure. The grant will also enable NCHC to coordinate food access for community residents who have been affected by the COVID-19 crisis.

About the Grantee: The Visiting Nurse Association Health Group is a nonprofit organization dedicated to helping individuals and families achieve optimal health and well-being by providing compassionate, coordinated, and innovative care in their homes and communities.
