The Urban Health Institute (UHI) consolidates outpatient internal medicine and specialty medical offices under one newly renovated space at Cooper University Healthcare. The UHI, which annually provides 32,000 outpatient safety net visits, is developing an advanced model of multi-specialty, integrated outpatient healthcare delivery focused on improving primary and specialty care and reducing the costs of care for Camden’s underserved (self-pay, Medicaid, and Medicare) populations. The model includes redesigning patient visits to make them more efficient and productive; implementing and scaling group visits; reducing no-shows and improving timely access through open access scheduling; engaging medical students, residents, and fellows to maximize the efficient use of health workforce resources; and providing hospitalized patients with care management to link them to primary care upon discharge.
The project supports UHI’s organizational infrastructure to facilitate the continuing development of this innovative model of healthcare for vulnerable, underserved populations.
