New Jersey Safety Net Population Performance Measures

Discern Health is a consulting firm that works with organizations across the private and public sectors to identify and carry out solutions to healthcare challenges. It focuses on projects in three main areas: care delivery, payment methods, and measurement.

The project supports Discern Health and its subcontractor, Social & Scientific Solutions, in employing evidence-based data analytic methods to examine baseline performance and costs of New Jersey’s safety net primary care services. Findings from this analysis will be used to develop recommendations to The Nicholson Foundation, the New Jersey Department of Human Services, and the New Jersey Department of Health on ways to identify and prioritize care delivery improvements for the state’s ambulatory healthcare safety net delivery system.
