The Center for Health Education, Medicine, and Dentistry (CHEMED), a division of the Lakewood Resource & Referral Center, is a Federally Qualified Health Center in Lakewood, New Jersey.
This project supports efforts to enhance CHEMED’s primary care services by incorporating an evidence-based integrated behavioral health model developed by Cherokee Health Systems into their delivery system. This model is used in conjunction with web-based cognitive behavioral therapies developed by Cobalt Therapeutics. The project began with a successful pilot project and is being continued and expanded with additional staffing and a systematic assessment of improvements in health outcomes as a result of using this care model.
