The Visiting Nurse Association of Central Jersey (VNACJ) Community Health Center is a Federally Qualified Health Center that provides comprehensive primary and preventive healthcare and wellness services to low-income New Jersey residents of all ages at four locations in Monmouth County.

This project supports the adoption and implementation of the Cherokee Health Systems integrated, team-based physical and behavioral health care model. In this model, licensed behaviorists serve as Behavioral Health Consultants (BHC) work as a part of a primary care team. 

Cherokee Health Systems has developed specific templates in NextGen, their Electronic Medical Record (EMR), for efficient and appropriate documentation of BHC services. VNACJ also uses NextGen, and this project integrates the Cherokee templates into the VNACJ NextGen platform. Successful implementation of these templates will greatly enhance the capacity of the VNACJ BHCs to accurately and efficiently document the care they provide.
