The Healthy Greater Newark Accountable Care Organization (ACO) is a group of Newark-area hospitals, primary care practitioners, specialists, behavioral health providers, and community organizations committed to improving the quality of care for local residents enrolled in Medicaid while controlling the overall cost of care. Healthy Greater Newark ACO is one of three such organizations approved by the State of New Jersey to form a Medicaid ACO as part of a three-year demonstration project.

This project supports the successful implementation of the Healthy Greater Newark ACO by providing funding for administrative leadership and for data analyses that integrate Medicaid claims data with clinical data in order to direct quality improvement and guide care team deployments. The project also supports the development of a plan to guide a care management program focused on patients who are particularly high users of Medicaid services.
