The Freehold Neighborhood Connections to Health Coalition is a collaborative effort of community organizations aimed at improving the health of the underserved in Freehold Borough. Participating members of the Coalition include the Visiting Nurse Association Health Group, the Freehold Area Health Department, CentraState Medical Center, Casa Freehold, Freehold Center Management Corporation, New Beginnings Agape Christian Center, and other community agencies.

This project supports the development of the Coalition’s infrastructure and capacity so that it can carry out evidence-based interventions to reduce health disparities and improve the health and well-being of community residents. An initial priority of the Coalition is to reduce the incidence of elevated blood levels in the community’s children and protect them from further exposure to lead.
