The Trenton Health Team (THT) is a community health improvement collaborative serving the six zip codes of Trenton, New Jersey. It is one of three such organizations approved by the State of New Jersey to form a Medicaid Accountable Care Organization (ACO) as part of a three-year demonstration project. The THT is a partnership between 50 community organizations, including St. Francis Medical Center, Capital Health, Inc., Henry J. Austin Federally Qualified Health Center, and the Department of Health of the City of Trenton.

This project continues support for activities that enhance THT’s ability to implement a successful Medicaid ACO. The project provides funding for administrative leadership and for efforts to improve THT’s capacity to integrate claims data with clinical data. The project also supports THT efforts to leverage evidence-based models to improve the operations of care management teams, particularly those focused on the care of patients who are high users of Medicaid services.
